Based on our experience with cross-sectorial collaboration with the municipality we have organised an interdisciplinary geriatric team to facilitate early discharge and offer hospital-at-home to the elderly acute medical patients. Intervention consists of active treatment in the patient’s home for a condition that otherwise would require acute hospitalisation. The aims are to reduce readmissions, to secure a high patient satisfaction, improve survival and reduce the health care cost.
